In accordance with Texas Health & Safety Code §181.105, the following information explains how to request your health care records, contact the Texas Behavioral Health Executive Council, and file a consumer complaint with the Texas Attorney General.
Requesting Your Health Records
You have the right to request access to your health records.
To request records from Finding Peace Counseling Services, PLLC, please submit your request in writing to:

Please include enough information for us to identify the records you are requesting and how you would like them delivered.
Requests for records will be handled in accordance with applicable Texas and federal law. In some circumstances, additional authorization may be required before records can be released to another person or organization.
For couples, families, or other clients who participated in services together, additional consent or review may be necessary before complete records can be released.

Texas Behavioral Health Executive Council
The Texas Behavioral Health Executive Council oversees the licensing and regulation of behavioral health professionals in Texas.
Consumers may contact the Council with questions or concerns regarding a licensed behavioral health professional.
